Below are the required documents to be eligible for the Nurse Aide program at Northern Pennsylvania Regional College.
- Tuberculin 2-step Test Form
- Verification of PA Residency Test Form
- Act 14 Attestation of Compliance Form
- Health Examination Form (Physical)
- How to obtain FBI Report
- Complete the request electronically for Pennsylvania Access to Criminal History
- FERPA Release - Fill out the linked form; it does not need uploaded.
