All documents must be submitted to be eligible to register.

Below are the required documents to be eligible for the Nurse Aide program at Northern Pennsylvania Regional College. Each document has a separate upload link. Please ensure you are uploading each document to the correct form. 

- Tuberculin 2-step Test Form

- Verification of PA Residency Test Form

- Act 14 Attestation of Compliance Form

- Health Examination Form (Physical)

- How to obtain FBI Report

- Complete the request electronically for Pennsylvania Access to Criminal History
